Northbeam Health runs twenty-two primary care clinics. Patients filled in a four-page paper form in the waiting room and staff typed it up while the doctor waited. We tested the paper form with eleven patients, then designed an intake that runs on the patient's own phone the night before: one question per screen, big tap targets, consent explained in plain language.
On the clinic side a dashboard shows which sections are done before the patient arrives. Eighty-two percent of patients now complete intake on their phone, and appointments start an average of six minutes earlier.
